  • The entire evaluation takes approximately 2 hours broken into two appointments, plus questionnaires.

  • Major insurances cover the first and second appointment, however not all insurances cover the QB Check Test. Please email us to find out more.

  • Yes. Because our evaluation combines a formal psychiatric clinical assessment with FDA-cleared objective data, it provides a robust medical record. This is often far more persuasive for securing 504 Plans, IEPs, or workplace accommodations than a simple questionnaire.

How The Process Works and Cost

Our thorough ADHD testing is broken up into 2 appointments + questionnaires.

  1. Pre Appointment questionnaires:

    1. We ask you to self report your symptoms as well as a family member or spouse through online questionnaires.

    2. if ages 6-12 We ask a parent and teacher to fill out online questionnaires.

  2. Appointment 1:

    1. Clinical Assessment $Covered under major insurance carriers

    2. QB objective computer test $250 if not covered or reimbursed

  3. Appointment 2:

    1. DIVA 5 Interview $Covered under major insurance carriers

  4. Post appointment analysis, results and write up, Accommodations and Sending of results
