The entire evaluation takes approximately 2 hours broken into two appointments, plus questionnaires.

Pre Appointment questionnaires:
We ask you to self report your symptoms as well as a family member or spouse through online questionnaires.
if ages 6-12 We ask a parent and teacher to fill out online questionnaires.

Clinical Assessment $Covered under major insurance carriers
QB objective computer test $250 if not covered or reimbursed
Appointment 2:
DIVA 5 Interview $Covered under major insurance carriers
Post appointment analysis, results and write up, Accommodations and Sending of results
