For general questions regarding scheduling, billing, pharmacy, prior authorization or documentation you can reach the main office at 877-779-2873 from 9:00am - 5:00pm EST Sun - Thurs. You can also reach your individual prescriber from this phone tree.

For current patients, your prescriber has most likely already reached out via text or email. If you wish to initiate direct conversation with your provider, you can use your patient portal. From there your prescriber can contact you via secure chat, as well as text or email.

For medical records, prior authorizations or if you are another medical office, fax us at 949-864-3159.

If you need to reschedule or cancel an appointment, please use our main number to contact your provider directly.
